Step 1 – Gather supporting documentation
When you request from USCIS a new certificate that proves your citizenship, you will have to provide the documents that support your request. Because you may want a new certificate for different reasons, your supporting documentation may include one of the following documents:

A photocopy of the original certificate, if applicable.
If you lost your certificate, you may still have a photocopy of it. USCIS will want that copy.

The original Certificate of Naturalization or Certificate of Citizenship.
If your certificate was impaired, or if there was a mistake made by USCIS, you will have to provide the original certificate.

A marriage or divorce decree, or a marriage annulment decree.
These documents are usually necessary to prove that your name was changed.

A court order or a birth certificate.
If your birth date was corrected, or if your original certificate has the wrong date, you will need a court order or birth certificate that proves you changed your birth date through legal action.

A court order or a medical certificate.
If you changed your gender after you received the certificate, USCIS will require a medical certificate or court order to verify if you gender was truly changed.

Once you have the minimal set of documents, you can move to the step 2 – filling out the form.
